Help for members who have lost or may lose health coverage

The “Life Saver Plan” is for members who may not work enough hours to maintain health coverage and can’t afford the full self-pay rate or COBRA. If you are struggling to maintain your health benefits or have lost coverage since January 1, 2009, you now have an affordable option to get basic health services.

For just $75 per month, the "Life Saver Plan" provides basic health services to you and your family.

The "Life Saver Plan" ONLY covers doctor visits, lab tests and prescription medications. Other benefits such as, dental, disability, Emergency Room, hospitals, chiropractor and vision are not covered.

For complete details or to enroll in the Life Saver Plan, please call the Culinary Health Fund Customer Service Office at:
702-733-9938 or visit the website  www.culinaryhealthfund.org.
